Why is Exposure and Response Prevention Therapy so Effective?

By Antoinette Peterson

Exposure and Response Prevention Therapy (ERP) is a type of Cognitive Behavioral Therapy used widely to treat Obsessive Compulsive Disorder (OCD). It is also commonly used to treat phobias, panic disorders, Post-Traumatic Stress Disorder (PTSD), social anxiety, and generalized anxiety disorder. ERP works by helping clients gradually face the thoughts, situations, or images that trigger anxiety or fear without resorting to an unhealthy response.  With the help of a trained therapist, clients learn to tolerate uncertainty and discomfort in manageable steps. But what exactly makes this process so effective? In this blog post, we will discuss the reasons why ERP works.

Why is ERP Effective?

As stated above, ERP is a commonly used treatment for OCD and other disorders. More generally, its efficacy comes as a result of breaking the conditioned response between obsessive thoughts and compulsions/avoidance. ERP primarily helps in the following ways:

    • Emotional processing: Through exposure, a client learns to attach more realistic beliefs about their fears, and can become more comfortable with the experience of fear.
    • Habituation: A client’s repeated exposure to their fears leads to a decrease in  their reaction to such stimuli over time.
    • Extinction: Exposure can weaken negative, learned associations with certain stimuli and bad outcomes.
    • Self – Efficacy: ERP shows the client they are capable of confronting their fears and managing anxiety.

ERP and OCD

More specifically, when evaluating the efficacy of ERP against other treatments for OCD, ERP stands out as it builds resilience against anxiety, fostering long term relief from OCD. Additionally, other forms of treatment such as Cognitive Therapy can be used in combination with ERP to supplement and improve treatment for those with impaired insight into their OCD. ERP can be used in both outpatient settings and intensive treatment programs, making it a viable option for clients with varying needs. Additionally, ERP addresses the root cause of OCD by disrupting the avoidance and compulsion cycle, making it more effective than treatments that aim for temporary calmness.
